Wmo (Social Support)

There’s already a lot available for you in Moerdijk. For example,there are all kinds of activities at the community center,volunteers are available to offer support, and support for family caregivers is available through the Mantelzorg Moerdijk website. You can find a lot of information onthe Moerdijk Voor Elkaar website. Perhaps that has already been helpful to you? If not, and you need further support, you may be eligible for assistance from the municipality.

Do you need assistance? Please contact theWmo help deskat the Municipality of Moerdijk. We’ll work with you to assess your personal situation and explore possible solutions.

Our team will first help you find a solution on your own. Sometimes this involves reaching out to people in your immediate circle or, if necessary, with the help of volunteers. You can read more about this onMoerdijk Voor Elkaar. If this does not provide a solution, a (customized) service under the Social Support Act (Wmo) may be an option. The Social Support Act (Wmo) is designed to ensure that everyone with a disability can continue to live independently and participate in society for as long as possible.

Are you facing obstacles in your home due to your disability? And are you unable to resolve the situation even with help from others? If so, please contact the municipality of Moerdijk. The municipality’s Wmo consultant will work with you to explore your options.

Shorter period

You can borrow assistive devices that you need for up to 6 months—such as a wheelchair—from a home care supply store.

Longer period

You can apply to the municipality of Moerdijk for services that you will need for a longer period (more than 6 months). To do so, please contact theWmo help desk.

No reimbursement

There are assistive devices that are no longer covered by insurance. These are devices that are not exclusively intended for people with disabilities, such as faucets, toilet bowls, wall-mounted grab bars, shower chairs (freestanding or wall-mounted), spa toilets, and threshold ramps. These devices are available on the open market.

Out-of-pocket cost

For most services or support provided under the Wmo, you are required to pay a statutory copayment. You will never pay more than the actual cost of the support or service.

The Central Administration Office (CAK) collects and determines this copayment. In some cases, you do not have to pay a copayment. You can find more information on the CAK website.

The suppliers that provide assistive devices—such as a patient lift or a mobile shower chair—through the Wmo are:

If your disability or illness prevents you from traveling independently, or if you find it difficult to get around in and around your home—making it impossible, for example, to go grocery shopping—please contact the Wmo help desk at the Moerdijk municipality. We’ll work with you to assess your options and discuss your situation. We’ll consider what you’re able to do on your own and whether people in your community can help. There may be local services you can use, such as a grocery bus or a delivery service. If these options don’t meet your needs, a customized solution may be available.

Getting Around in and Around Your Home

If you’re having trouble getting around in and around your home, a wheelchair might be helpful. You can borrow a wheelchair for free from a home care store—whether it’s for a day out or for up to 6 months.

You do not have to paya copaymentfor a wheelchair. 

Getting Around in Your Immediate Area

Don’t have your own transportation? Is it not possible for you to travel by public transportation? And do you lack sufficient support from family, neighbors, and friends? If so, a shared taxi pass may be an option for you. With this pass, you can use the shared transportation system at the public transportation rate. To apply, please contact theWmo help desk.

A mobility scooter or tricycle might also be helpful. You may be eligible for one if you cannot walk long distances and are also unable to ride a bicycle or use a moped or light moped.

There are assistive devices that are no longer covered. These are devices that are not offered exclusively to people with disabilities, such as electric bikes, low-step bikes, cargo bikes, or trailer bikes. These devices are available on the open market.

For an individual transportation service, such as a tricycle or mobility scooter, you mustpayacopayment. For theDeeltaxi, you pay the applicable fare on site.

If you are eligible for a medical device, you can choose your own supplier.

Moving Outside Your Region

Do you need transportation outside the region (that is, approximately 25 km or more from your home)? If so, you can apply for a Valys pass. Valys provides transportation outside the region for travelers with limited mobility. For more information, visit the Valys website.

Do you have trouble keeping your household in order? Maybe getting some help with this could be the solution for you.

One solution, for example, could be to get help from family or friends, or to hire private assistance yourself. There are often easily accessible solutions available for your problem. A Wmo consultant from the municipality of Moerdijk will work with you to explore all your options. These might include using a grocery delivery service, childcare, or a meal service. If this does not provide a solution, you may be eligible for household assistance from the municipality. This includes tasks such as washing windows, vacuuming, and cleaning the bathroom.

If you are eligible for home care services, you can choose your own care provider.

Do you need help with (general) daily activities? This may include planning, encouraging, and preparing activities such as:

  • Establishing a Daily Routine
  • Social Connections and Social Environment
  • Gaining insight into the potential consequences of decisions
  • Preparing for conversations about housing, education, work, income, and buying or paying for things
  • applying skills learned during treatment sessions
  • Support with administrative tasks, mail, and finances
  • a meaningful way to spend the day

People around you are often willing to help. This includes family members and neighbors, as well as local services you can use. For example, you could go have a cup of coffee at a community center. Or perhaps you could volunteer. If this does not sufficiently address your need for assistance, you may be eligible for services under the Wmo.

To do so, please contact theWmo service deskat the Municipality of Moerdijk. The Wmo counselor will work with you to explore your options.

Are you receiving informal care and would you like to ease the burden on your caregiver? If so, you may be able to attend a day care center temporarily or stay at a facility or respite home. This support is designed to relieve the burden on the informal caregiver or the person’s immediate family. It may be intended for clients who require constant supervision or who need ongoing care or care at irregular intervals.

Some health insurers cover this “respite care” through supplemental insurance. If this is an option, it takes precedence over support provided under the Wmo.

You are required to pay acopaymentfor this respite care. 

Do you encounter difficulties in your daily life that prevent you from participating (or fully participating) in the community? If so, we’d be happy to work with you to find a suitable solution. During a “kitchen-table discussion” between you and a consultant from the municipality of Moerdijk, we’ll discuss your personal situation and the options available to you, with support from your personal network. We’ll also discuss the options for informal and professional support, as well as any necessary services.

The results of the “kitchen-table discussion” can vary from person to person. We want the support to be tailored to your personal situation and needs. In other words, it’s customized! This way, as a resident, you’ll get exactly what you need.

If the kitchen-table discussion and the assessment by the Wmo consultant show that you are eligible for an individual Wmo service, the municipality will provide you with a customized service “in kind” (ZIN). This means that the municipality will purchase the support or service on your behalf.

In some situations, it is also possible to opt for a personal budget (PGB). You will then receive a sum of money that you can use to purchase the support or services you need yourself. You choose the care provider yourself and enter into a care agreement with them.
Keep in mind, however, that the PGB will never exceed the maximum rate for comparable care provided in kind. Therefore, you will often have to pay an additional amount out of pocket to purchase the care or service.

You should choose a PGB if:

  • Personalized care or a customized service is not available through in-kind care.
  • You want to—and are able to—take charge yourself. You choose your own healthcare provider or service provider.
  • Whether the care cannot be planned properly in advance, must be provided at unusual times, or requires 24-hour on-call support.
  • You must receive care from a designated caregiver due to the nature of your disability.

PGB Requirements:

  • You will work with your healthcare provider to draw up a budget plan and a care agreement.
  • You are able to manage your care yourself or with the help of an informal caregiver. The service provider may not be paid from the PGB to take over this management role from you.
  • There is no debt restructuring or court-appointed administration involved.
  • The copayment, imposed by the Central Administration Office (CAK), may not be paid from the PGB.
  • The municipality applies different rates for care provided by a professional caregiver or by a friend, neighbor, or family member.
  • You are personally responsible for the maintenance and repair of equipment purchased with a PGB.

PGB Payment:

  • PGB funds are not paid directly to clients. The government has designated the Social Insurance Bank (SVB) to administer the PGB funds and disburse them to the provider(s) with whom you have entered into an agreement. To this end, you must submit your care agreement to the SVB. Your care provider will send their invoice directly to the SVB in order to receive payment.
