Medical Examination and Health Declaration

You may be required to undergo a medical examination when applying for or renewing a driver's license. You will need a medical certificate for this examination.

You must undergo a medical examination when applying for or renewing a driver's license if:

  • Your current driver's license expires on or after your 75th birthday;
  • You have a driver's license with limited validity (for example, due to a medical condition);
  • When obtaining or renewing a driver’s license in categories C, CE, C1, C1E, D, DE, D1, or D1E;
  • When obtaining a tractor driver's license (T license), a motorcycle license, or a car driver's license (possibly with a trailer);
  • When exchanging a driver’s license from the former Netherlands Antilles or another country with which the Netherlands has made arrangements for such an exchange. You can find the list of countries on the RDW website.

You will need a health certificate for the medical examination. The Municipality of Moerdijk only sells paper health certificates for individuals aged 75 and older. You can purchase the other health certificates online on the CBR website.

Requests

You can obtain a health certificate in the following ways:

  • My CBR for purchasing and filling out a Health Declaration online. You can log in to My CBR using DigiD with SMS verification, and payment is made via iDeal;
  • The CBR online store for a paper Health Declaration. This costs an additional 10 euros. The declaration can be ordered without a DigiD and will be mailed within 5 business days. Payment is made via iDeal;
  • The CBR testing centers in Amsterdam, Arnhem, Eindhoven, and Rijswijk for a paper Health Declaration;
  • Apply online with the municipality for the 75+ Health Declaration.
  • Or make an appointment with the city.

Long wait times for medical evaluations

The CBR is very busy right now. That’s why it’s a good idea to fill out the Health Declaration on time, but not too early—four months before your driver’s license expires. Do you have any health issues? In that case, the CBR recommends that you fill out your Health Declaration five months in advance, because as part of the assessment, you will likely be referred to a doctor or specialist.

Unfortunately, the CBR cannot assist you any sooner if you submit the declaration earlier. Submitting the Health Declaration digitally can speed up the process. You can also check the status of your application at any time online via Mijn CBR.
