Reporting a Death

A death must be reported within 6 business days of the death. In any case, this must be done before the funeral. Usually, the funeral director handles the report. As a next of kin, you can also report a death yourself. You must do this at the municipality where the person died. You’ll need to make an appointment to do so.

Funeral directors can report a death online to the municipality of Moerdijk. To do so, the funeral director must have an eHerkenning account and be registered with us as a digital reporter.

Report Filed by a Surviving Relative

You can also file a death certificate by appointment. Please bring the following documents to your appointment:

• A B-envelope containing the doctor's form regarding the cause of death
• Certificate of Death (issued by a medical examiner)
• the marriage certificate, if applicable
• valid identification of the person filing the report

The cremation or burial must take place no later than the sixth business day after the death. If you need to extend this deadline, please also bring:

• a statement from a physician confirming that there are no objections to postponing the burial or cremation.

In the case of an unnatural cause of death, the following is also required:

• Certificate of Unnatural Death (this is sent directly to the municipality by the medical examiner)
• Certificate of No Objection for the burial or cremation of the public prosecutor (this is sent directly from the Ministry of Justice to the municipality).
